material changes are made, patients will be notified in writing or via prominent posting.
8. Complaints
If you have complaints regarding the way your protected health information was handled, you may submit a complaint in writing to
our office. You will not be retaliated against in any manner for a complaint.

Antioch Optometry SMS Messaging Program

Program Description
The Antioch SMS Messaging Program is used to communicate with patients regarding appointment reminders and confirmations, digital intake form notifications, billing messages and invoices, and appointment rescheduling assistance. Messages may include links and phone numbers to help patients complete forms, secure payment links to pay, or contact the office.

Cancellation / Opt-Out
You can cancel the SMS service at any time. To unsubscribe, simply text the keyword STOP to 925-470-4616. After you send the SMS message STOP to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time or text START to the same number, and we will start sending SMS messages to you again.
